Volunteer Service Agreement

All participants in City of San Rafael Volunteer events are required to complete a Volunteer Service Agreement.

These forms may be printed and completed at home, and will also be provided on site.

If a volunteer is under the age of 18, the form must also be signed by a parent/guardian.
