City of San Rafael Municipal Code Chapter 8.34 REGULATIONS FOR MASSAGE AND/OR BODYWORK OFFICES AND ESTABLISHMENTS, OWNERS, AND PRACTITIONERS
Procedure:
Complete the application below and attach the following information:
1. California driver’s license, if any, or other photographic identification issued by a state or United States federal agency establishing the applicant’s age as eighteen (18) or older.
2. Two (2) recent portrait photographs that clearly and accurately depict the applicant’s current facial features.
3. Proof of completion of 100 hours at a certified school of massage, original to be reviewed by staff.
4. Certificate and certified transcript from massage school, original to be reviewed by staff.
5. The permit history of the applicant for the five-year period immediately preceding the date of the filing of the application, including whether the applicant has operated under any other business name in the city or has ever had any similar license or permit revoked or suspended or has been convicted of a violation of this chapter, and if so, the circumstances of such suspension, revocation or conviction. Copies of previously issued Permits or Licenses from other cities or counties.
6. If business, incorporation form showing all owner's names, all employee names and form below signed by landlord to show property owner knowledge of business operation.
7. Verification of home address, a utility bill is sufficient.
8. Schedule and complete a fingerprint check at the San Rafael Police Department.
9. Schedule and complete a written multiple choice test of massage and/or bodywork knowledge at the San Rafael Police Department.
10. Payment for the permit fee in an amount established by the City Council.

For this permit you must obtain a City Business License in addition to the Massage/Bodyworker Permit. Follow this link to determine what type of business license you require.
For this permit, your business location must be pre-approved by the City of San Rafael Planning Department. Take the completed packet to the Community Development Department on the Third Floor of City Hall to determine if the location you have selected is zoned for the business you wish to conduct prior to making an appointment with the SRPD Regulatory Services Officer.
Fees:
The current fee for this permit is $180.00 and $35 for a renewal or location change permit. Each additional owner is $89.00 and each additional location is $71.00 There will be an additional charge of $76.00 for fingerprinting for this permit.
