- Reporting Party Name, Phone Number, and Address are required for all complaints submitted.
- Check the box under Address if you wish to be updated with the course of action.
- Anonymous complaints will not be investigated.
- Reporting party information is strictly confidential and never released.
- Complaint forms can be submitted by fax, mail, or in person at City Hall.
